NGO · WASH program

Baseline survey for a water and sanitation program

Problem

An NGO launching a WASH program across several woredas had a 60-question paper baseline. Previous paper surveys had produced inconsistent household IDs, missing responses, and weeks of manual data entry.

Solution

We rebuilt the instrument as an advanced KoboToolbox form: automatic household ID generation, GPS capture at each interview, constraints on ages and quantities, and cascading location selects so enumerators could not misfile a household.

Impact

Data entry disappeared as a task. Supervisors reviewed submissions the same evening instead of after fieldwork ended, and the final dataset went to the analyst without a cleaning backlog.

PhD field research form with complex skip logic

Problem

A PhD candidate studying maternal health needed a questionnaire where entire modules depended on earlier answers: pregnancy history, facility visits, and referral outcomes. Piloting on paper, enumerators kept skipping to the wrong module.

Solution

We translated the protocol into an ODK form with layered relevance conditions, repeat groups for each pregnancy, and built-in consistency checks between dates. Every path through the form was tested with scripted scenarios before the pilot.

Impact

The routing errors vanished from the pilot data. The candidate could show the ethics committee and supervisors a documented, tested instrument, and analysis started on schedule.

Monitoring tool for a government program

Problem

A government project needed quarterly monitoring data from field offices in multiple regions. Each office had its own spreadsheet format, so every quarter began with weeks of reconciling incompatible files.

Solution

We designed a standardized SurveyCTO monitoring form with indicator calculations built in, plus a short training session and a reference guide so every office submitted data the same way.

Impact

Quarterly reporting became a single consolidated export. Program managers spent their time reviewing indicators and asking better questions instead of fixing spreadsheets.
